The clinical manifestations of the disease include homogentisic aciduria (presenting in infancy), gradual development of ochronosis after 20–30 years of age, and degenerative ochronotic arthropathy, typically occurring in the fourth decade of life. In addition, involvement of the cardiovascular system and the urinary tract may be observed. Management of ochronosis requires a comprehensive approach that includes both pharmacological and non-pharmacological interventions. Reports of clinical cases of ochronosis in the Russian scientific data remain limited.</p> <p><bold>CASE DESCRIPTION: </bold>This article presents a clinical case of ochronosis with involvement of the cardiac valvular apparatus and the musculoskeletal system in an 80-year-old woman with a history of comorbidities. Musculoskeletal condition had been present since 2007. Pronounced cutaneous manifestations appeared in 2023 (pigmentation changes of the auricles, cheeks, and sclerae). The patient underwent multiple surgical interventions for musculoskeletal disorders. In 2023, targeted laboratory investigations were performed for the first time to confirm ochronosis: 3-methylcrotonylglycine, 8.37 mmol/mol; homogentisic acid, 95.93 mmol/mol creatinine; and p-hydroxyphenyllactic acid, 6.24 mmol/mol creatinine, thereby confirming the diagnosis. Modern imaging and laboratory methods were key to the accurate diagnosis of this condition.</p> <p><bold>CONCLUSION: </bold>Improving awareness of ochronosis among physicians of various specialties is essential for its early detection. Timely diagnosis of ochronosis enables initiation of treatment at earlier stages, which may reduce the risk of complications and improve patients’ quality of life.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование. </bold>Охроноз — редкая патология, обусловленная врождённым нарушением обмена аминокислоты тирозина, процесс биохимического превращения которой останавливается на этапе образования гомогентизиновой кислоты. Клинические проявления заболевания: гомогентизиновая ацидурия (проявляется в младенческом возрасте), постепенное развитие охроноза после 20–30 лет и дегенеративные охронозные артропатии, обычно на четвёртом десятилетии жизни. Кроме того, встречаются поражения сердечно-сосудистой системы и мочевыводящих путей. В терапии охроноза необходим многосторонний подход, охватывающий как медикаментозные, так и немедикаментозные методы воздействия. Описаний клинических случаев охроноза в отечественной литературе немного.</p> <p><bold>Описание клинического случая. </bold>В нашей статье представлен клинический случай охроноза с поражением клапанного аппарата сердца и костно-мышечной системы у женщины 80 лет. Пациентка с коморбидным анамнезом. Патология опорно-двигательного аппарата с 2007 г. Выраженные кожные проявления с 2023 г. (заметные изменения на ушных раковинах, щеках, склерах). Больной проведены неоднократные оперативные вмешательства по поводу патологии опорно-двигательного аппарата. В 2023 г. впервые были выполнены анализы с целью подтверждения охроноза: 3-метилкротонилглицин — 8,37 ммоль/моль, гомогентизиновая кислота — 95,93 ммоль/моль креатинина, парагидроксифенилмолочная кислота — 6,24 ммоль/моль креатинина, таким образом диагноз охроноза был подтверждён. Современные визуализирующие и лабораторные методы сыграли важную роль в точной диагностике данной патологии.</p> <p><bold>Заключение. </bold>Важно улучшить информированность об охронозе среди врачей разных профилей для раннего выявления этого заболевания.
